* Change of materials including type and quantity
* New information being made available
* Change to legislation and guidance
* Different people being involved or likely to be harmed (e.g. vulnerable people)
* As a response to an accident or near miss
* Following an audit or review
* When the process does not function as expected
* At regular intervals (e.g. annually)
